The Role

Provide comprehensive, patient-centered care to older adults with cognitive impairment ranging from MCI to advanced dementia. You will follow patients and families longitudinally, serving as a trusted clinical partner throughout diagnosis, progression, and care transitions.

Key Responsibilities

Cognitive Assessment & Diagnosis

  • Conduct comprehensive cognitive evaluations
  • Order and interpret diagnostic testing (labs, imaging, neuropsychological assessments)
  • Communicate diagnoses with clarity, compassion, and sensitivity

Ongoing Cognitive & Symptom Management

  • Develop and manage individualized, evidence-based care plans across all stages of dementia
  • Partner with caregivers and care teams on behavioral and symptom management strategies

Functional & Safety Assessment

  • Assess ADLs, IADLs, mobility, fall risk, and home safety
  • Recommend supportive services, assistive devices, and environmental modifications

Care Coordination & Clinical Leadership

  • Coordinate care across primary care, neurology, psychiatry, social work, therapy, and community resources
  • Serve as a clinical escalation point for the interdisciplinary care team

Care Partner Education & Support

  • Educate caregivers on disease progression, symptom management, and realistic expectations
  • Provide support during crises, transitions, and emotionally complex situations

Advance Care Planning & Future Preparedness

  • Lead proactive planning to reduce avoidable hospitalizations and crisis escalation
  • Facilitate goals-of-care discussions, advance directives, POLST forms, and surrogate decision-making

Qualifications

  • Active, unencumbered RN and APRN licensure in the state
  • 3+ years of experience in geriatrics, neurology, dementia care, or related fields
  • Relevant certification (PMHNP, AGNP, GS-C, or similar) preferred
  • Strong communication skills with cognitively impaired patients and families
  • Comfort with longitudinal care, caregiver dynamics, and end-of-life discussions
  • A collaborative, thoughtful approach to complex care
