Nurse Practitioner – Dementia & Geriatric Specialty

Practice at the top of your license. Build relationships. Change how dementia care is delivered.

Do you want the time and support to truly care for older adults living with cognitive impairment—and the families who love them?

Are you looking for a role where your clinical judgment is trusted, your schedule is sustainable, and your work has deep meaning?

We partner with primary care practices to deliver comprehensive dementia care — focused on early diagnosis, thoughtful planning, caregiver support, and longitudinal follow-up across the disease course.

Why Nurse Practitioners Choose Us

  • Autonomous, NP-led practice with physician collaboration and specialty support
  • Daytime, weekday-focused schedule with flexibility and rare after-hours accommodations
  • Hybrid role with meaningful work that can be done from home
  • Longer visits and continuity of care—quality over volume
  • Robust clinical support, including care coordinators, streamlined systems, and dementia-specific workflows
  • Mentorship and specialty expertise in dementia and palliative care
  • Clear growth pathways into team leadership, mentorship, and program development roles

The Role

Provide comprehensive, patient-centered care to older adults with cognitive impairment ranging from MCI to advanced dementia. You will follow patients and families longitudinally, serving as a trusted clinical partner throughout diagnosis, progression, and care transitions.

Ongoing Cognitive & Symptom Management

  • Develop and manage individualized, evidence-based care plans across all stages of dementia
  • Partner with caregivers and care teams on behavioral and symptom management strategies

Functional & Safety Assessment

  • Serve as a clinical escalation point for the interdisciplinary care team

  • Provide support during crises, transitions, and emotionally complex situations

Advance Care Planning & Future Preparedness

Qualifications

  • Active, unencumbered RN and APRN licensure in the state
  • 3+ years of experience in geriatrics, neurology, dementia care, or related fields
  • Relevant certification (PMHNP, AGNP, GS-C, or similar) preferred
