Nexus Neurorecovery Center – Conroe is seeking an experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our interdisciplinary clinical team caring for individuals with complex neurological, medical, and behavioral health needs.

This is not a traditional outpatient psychiatric medication-management role. Our patients may present with acquired or traumatic brain injuries, neurological conditions, chronic and acute medical needs, cognitive impairment, and complex behavioral or psychiatric diagnoses. The ideal candidate will bring strong psychiatric expertise along with a solid medical foundation and the clinical confidence to evaluate the whole patient.

The PMHNP works collaboratively with physicians, nursing, rehabilitation, behavioral health, and other members of the treatment team to assess patients, manage psychiatric and medical concerns within scope of practice, monitor treatment response, and support continuity of care throughout the patient's rehabilitation journey.

What You'll Do

In this role, you will:

  • Perform comprehensive psychiatric, medical, physical, and neurological assessments.
  • Obtain detailed patient histories and evaluate changes in medical, neurological, psychiatric, and behavioral status.
  • Assess and manage acute and chronic conditions within scope of practice and established clinical protocols.
  • Diagnose psychiatric conditions and develop individualized treatment recommendations.
  • Prescribe and manage psychiatric and other medications within applicable scope and prescriptive authority.
  • Order appropriate laboratory and diagnostic studies, interpret results, and initiate appropriate interventions.
  • Monitor patients for medication effectiveness, side effects, drug interactions, and changes in overall clinical condition.
  • Recognize changes in patient status requiring immediate intervention or physician consultation.
  • Perform emergency and life-saving procedures when clinically necessary.
  • Collaborate closely with physicians and other healthcare providers in managing complex and long-term patient needs.
  • Provide ongoing monitoring and continuity of care between physician visits.
  • Document assessments, progress notes, treatment plans, interventions, and patient response accurately and timely.
  • Participate in admissions meetings and weekly interdisciplinary treatment team meetings.
  • Educate patients, families, nursing staff, and other members of the care team regarding diagnoses, medications, treatment plans, and ongoing care needs.
  • Serve as a clinical resource to facility staff.
  • Participate in rotating medical call coverage as required.
  • Maintain compliance with applicable Texas Board of Nursing requirements, facility policies, infection prevention standards, and established standards of clinical practice.

What We're Looking For

We're looking for a provider who understands that psychiatric symptoms don't occur in isolation. The strongest candidate will be comfortable evaluating how medical conditions, neurological injuries, medications, psychiatric diagnoses, cognition, and behavior intersect in a medically complex rehabilitation population.

Required Qualifications

  • Master's degree in Nursing as a Nurse Practitioner or master's degree in Nursing with completion of a post-master's Nurse Practitioner certification program.
  • Current Texas Registered Nurse (RN) license.
  • Current Texas Advanced Practice Registered Nurse (APRN) licensure.
  • Current Nurse Practitioner certification appropriate to the position.
  • Current BLS certification and ability to maintain certification throughout employment.
  • Psychiatric Mental Health Nurse Practitioner certification/psychiatric advanced practice training appropriate to the responsibilities of the position.
  • Strong clinical foundation in medical assessment and management, including the ability to recognize and respond to acute changes in patient condition.
  • Ability to participate in required behavioral management and de-escalation training.

Preferred Experience

  • 3–5 years of Nurse Practitioner experience in a medically complex patient population.
  • Experience in inpatient, hospital, rehabilitation, long-term care, neurorehabilitation, or other medically intensive settings strongly preferred.
  • Experience treating individuals with traumatic or acquired brain injury, neurological disorders, neurobehavioral conditions, or complex behavioral health needs.
  • Demonstrated experience managing patients with both psychiatric and significant medical comorbidities.
  • Experience working as part of an interdisciplinary treatment team.
  • Behavioral management certification or training such as SAMA, ABA, PIP, or similar programs is preferred.
