Lighthouse Behavioral Health Hospital is a psychiatric hospital that began offering substance abuse and mental health services in 2002. Primarily established to house adolescents, Lighthouse has transitioned to offering a full spectrum of acute inpatient services to adults and seniors as well. Since 2010, Lighthouse has expanded its mental health and substance abuse services. In 2013, Lighthouse opened a 28-day inpatient chemical dependency rehabilitation unit—the first of its kind in the Grand Strand area. In 2015, Lighthouse opened an acute adolescent unit to meet the needs of teens and their families in the Grand Strand. In 2022, Lighthouse opened an Adult Substance Use Disorder Intensive Outpatient Program. At Lighthouse, we strive to perform at a high level for our patients and their families.

Position Overview - Step-Down Navigator

  

The Step-Down Navigator is responsible for meeting with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes.  He/ She will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed.  A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred. 

 

Responsibilities include, but not limited to:

  • Meeting with eligible patients to discuss the benefits of step-down services as part of their continuum of care treatment.
  • Communicating eligibility to senior leadership and associated departments.
  • Coordinating between departments to ensure a smooth transition for patients.
  • Conducting patient therapy groups

Qualifications

Requirements:

  • Education: High School graduation or GED required.  
  • Experience: Minimum two (2) years’ experience in an inpatient psychiatric/mental health setting.
  • Licensure: Peer Support Certified, preferred. Current CPR, Physical and Verbal Crisis Intervention Certification, preferred.  Training may be obtained during new hire orientation.
