Job description:

Concordia at Villa St. Joseph is seeking a compassionate and motivated Social Worker - Part time/Full time to join our care team. This is a fantastic opportunity for a professional who is passionate about serving the geriatric population and is looking for a role with flexibility, growth potential, and meaningful work.

The Social Worker - Part time/Full time provides support to residents and their families in accordance with Federal and State regulations, as well as facility policies, to ensure psychosocial needs are met and residents are supported with dignity and respect.

Qualifications of a Social Worker - Part time/Full time:

  • Bachelor’s Degree in Social Work or Human Relations field (sociology, counseling, psychology, special education, rehabilitation, etc.)
  • At least one year of supervised social work experience in a health care setting preferred but not required
  • Strong documentation skills, communication skills with residents, families, staff, and community partners and ability to work independently and as part of a team

 

Key Responsibilities of a Social Worker - Part time/Full timer:

  • Admissions: Support new residents and families during the admission process; complete social history, care plans, and discharge planning documentation.
  • Care Planning: Participate in interdisciplinary care conferences and update resident care plans and involve families in care planning, offer support, and provide guidance on community services.
  • Discharge Planning: Coordinate follow-up services and provide resources to residents and families.
  • Documentation: Complete timely and accurate notes, assessments, and reports in compliance with regulations.
