RiseWell Community Services is looking for a Licensed Practical Nurse for our Methadone Clinic in our Clinical Department.

In this role, you will be providing health management and counseling, in conjunction with the psychiatrist, registered nurse as a member of a treatment team for persons with substance abuse disorders. Ensure medical standards as per program regulations are being provided.

QUALIFICATIONS:Ā Currently licensed as a Licensed Practical Nurse by the New York State Education Department. A valid and clean NY State driver license. Minimum 1 year of experience in a treatment or rehabilitation setting preferred. Fingerprinting, criminal record check, approval from OASAS.

ESSENTIAL FUNCTIONS:


  1. Responsible for medication management, education and administration
  2. Responsible to attend assigned meetingsĀ 
  3. Responsible to conduct all assigned visits
  4. Responsible to oversee the accurate completion of all documentation
  5. Maintain compliance with incident reporting guidelinesĀ 
  6. Responsible to comply with agency Compliance Plan and False Claims Policy by preventing, detecting and reporting any potential healthcare abuse and fraud. Report any potential non-compliance and comply with all other federal, state and local regulations applicable to this job description.
  7. Responsible for proficiency in all agency based IT applicationsĀ 
  8. Provide education to clients and staff regarding standards of medical careĀ 
  9. Responsible to perform all other job duties as assigned
