Paid per visit ($75+ per visit) Flexible scheduling – part-time, full-time, or per diem opportunities Mileage & driving time reimbursement 3 weeks PTO per year One-on-one patient care Referral bonuses up to $500 Annual merit increases with performance evaluations Full-time benefits (if applicable): Medical, Dental, Vision, Life Insurance & 401(k) match after 1 year Your Role: Evaluate patients, develop individualized treatment plans, and administer therapy in the home Supervise COTAs and HHAs according to the treatment plan Chart patient progress, maintain accurate medical records, and submit notes timely Collaborate with families, caregivers, and interdisciplinary team Review and recommend adaptive equipment or home modifications as needed Maintain patient confidentiality per HIPAA regulations Coordinate total patient care and submit referrals for paramedical services Qualifications Qualifications: Licensed SLP Minimum 1 year of SLP experience preferred OASIS and WellSky (Kinnser) experience strongly preferred Physical exam within 12 months prior to first patient contact Current TB test (within 12 months) & CPR certification Valid driver’s license & proof of automobile insurance Ability to attend 3 day orientation at our office Join a team that values your expertise and empowers patient independence!
