BHCOR-Oncology Pathway I Corbin, KY
The Oncology Pathway team plays an important role in guiding and supporting patients throughout their cancer journey, from diagnosis through treatment, survivorship, and follow-up care. Team members work closely with physicians, nurses, navigators, and multidisciplinary clinical teams to coordinate care, reduce barriers, and help ensure patients have timely access to the services and support they need. This department is focused on delivering a compassionate, patient-centered experience while promoting continuity of care, quality outcomes, and strong collaboration across the oncology program.
The Breast Nurse Navigator is a registered nurse who provides comprehensive, patient-centered
navigation for individuals undergoing breast cancer screening, diagnosis, treatment, and survivorship.
The navigator serves as the primary clinical liaison, coordinating care across the multidisciplinary team, reducing barriers to timely treatment, providing education and emotional support, and promoting high quality, evidence-based care throughout the continuum
Description:
- Guides patients throughout their plan of care and connects them with appropriate healthcare and community resources.
- Provides education and navigation for breast cancer patients from diagnosis through treatment and survivorship.
- Assesses patient needs, support systems, and barriers to care.
- Communicates biopsy results and coordinates appropriate follow-up, treatment, and surgical consultations.
- Serves as a patient advocate, educator, clinical resource, and liaison among patients, families, physicians, and multidisciplinary teams.
- Supports cancer prevention, screening, community outreach, support groups, and quality improvement initiatives.
- Collaborates with Cancer Registry, Quality, physicians, and other clinical teams to promote continuity and evidence-based cancer care.
- Maintains Nurse Navigator program processes, documentation, confidentiality, and compliance with organizational policies.
Minimum Requirements:
- Associate degree from an accredited school of Nursing
- Current RN license in Kentucky or Kentucky work permit
- BLS – American Heart Association required within 90 days of hire
Preferred
- Bachelor’s degree
- One year of nursing experience preferred
- Previous Oncology experience preferred, but not required
