Full Time RN - Hospice

Job Summary

We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the appropriate medical services and support throughout their healthcare journey. This position requires a strong understanding of various medical settings.  

Duties

• Assess and evaluate patients' needs to develop individualized care plans.
• Coordinate with healthcare providers, patients, and families to facilitate effective communication and care transitions.
• Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
• Provide education and support to patients regarding their health conditions and treatment options.
• Maintain accurate documentation of patient interactions, care plans, and progress notes.
• Collaborate with interdisciplinary teams in various settings
• Utilize knowledge of medical terminology and case management principles to enhance patient care delivery.
• Advocate for patients' needs within the healthcare system while ensuring compliance with policies and regulations.

Qualifications

• Current Registered Nurse (RN) license in good standing.
• Experience in hospice care is preferred.
• Previous experience in case management is highly desirable.
• Strong understanding of medical terminology and healthcare systems.
• Excellent communication skills, both verbal and written.
• Ability to work collaboratively within a multidisciplinary team environment.
• Strong organizational skills with the ability to manage multiple priorities effectively.
