Opportunity

We’re hiring on behalf of a South Texas hospice provider seeking a full-time Hospice RN Case Manager to support patients across the Corpus Christi and Kingsville area.

This is a 7-on/7-off field-based position with no traditional daytime schedule during the working week. Coverage begins at 5:00 PM and includes routine hospice visits plus overnight on-call coverage.

The Corpus Christi territory currently has very low call volume, which is why routine visits are included during the on-week. After each 7-day working block, you’ll have 7 consecutive days off.

What the 7-On / 7-Off Schedule Looks Like

Monday–Thursday

  • Call coverage runs from 5:00 PM to 7:30 AM
  • Can include approximately 3 routine visits

Friday

  • Can include approximately 3 routine visits
  • Weekend call begins at 5:00 PM

Saturday & Sunday

  • Can include approximately 5 routine visits each day
  • Weekend call continues from Friday at 5:00 PM through Monday at 7:30 AM

Routine visits will most likely be facility patients, allowing them to be completed relatively quickly and generally no later than 8:00 PM.

Territory

  • Routine visits across Corpus Christi and Kingsville
  • On-call coverage is Corpus Christi only
  • Visits may take place in patient homes, nursing facilities, SNFs, assisted living communities, and other care settings
  • Reliable transportation and a valid driver’s license are required

What You’ll Do

  • Manage and support hospice patients during your scheduled on-week
  • Complete nursing assessments and routine field visits
  • Monitor symptoms, medications, and changes in condition
  • Develop and update individualized plans of care
  • Educate patients and families on symptom management, medications, disease progression, and end-of-life expectations
  • Coordinate care with physicians and the interdisciplinary hospice team
  • Respond to urgent after-hours patient needs during scheduled call coverage
  • Maintain accurate and timely EMR documentation

What We’re Looking For

  • Active, unrestricted RN license
  • Comfortable with the 7-on/7-off schedule and overnight call
  • Comfortable working independently in the field
  • Able to travel across both Corpus Christi and Kingsville
  • Reliable transportation and valid driver’s license
  • Strong clinical assessment and nursing judgment
  • Strong communication skills with patients, families, physicians, and care teams
  • Comfortable supporting patients and families through serious illness and end-of-life care
  • Able to maintain timely EMR documentation

Hospice Experience Is Preferred — Not Required

Strong transferable nursing backgrounds are welcome, including:

  • Home health
  • ER / ICU
  • SNF / long-term care
  • Case management
  • Field nursing
  • High-acuity or serious-illness care

If you are newer to hospice, you should have a strong clinical foundation, be comfortable working independently, and have a genuine interest in supporting patients and families through end-of-life care.

Compensation

$80,000–$83,000 annual base salary + benefits
