Job DetailsJob Location: ATEX Hospice - Corpus Christi - Corpus Christi, TX 78411Position Type: Full TimeGENERAL PURPOSE:  The LVN performs selected nurse care under the direction of the registered nurse for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient/family and referring agency.    QUALIFICATIONS:  Successful completion of a certified licensed practical/vocational nursing program.  Licensed to practice as a practical/vocational nurse in the state.  Minimum of six (6) months to one (1) year of clinical experience.  Six (6) months to one (1) year of previous home health or hospice experience preferred  Reliable transportation. Valid and current auto liability insurance  Must be a licensed driver with an automobile that is insured in accordance with state/or Company requirements and is in good working order.  Must hold a current and valid CPR certification.    ESSENTIAL JOB FUNCTIONS:  Under the direction of the RN, assist in identifying the patient’s physical, psychosocial, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations.  Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.  Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.  Assures the continuity of care through delivery of quality patient care.  Provides effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations.     Monitors assigned cases to ensure compliance with requirements of third-party payor.  Demonstrate commitment, professional growth, and competency.  Keep abreast of changes and trends affecting nursing care and licensed nurse functions.  Must be capable of performing the job functions of this position with or without accommodations.  Complies with acceptable professional standards and practice.    Adhere to all the Company’s policies and procedures.  Perform other duties as assigned.     The above statements are intended to be a representative summary of the major duties and responsibilities performed by incumbents of this job.  The incumbents may be requested to perform job-related tasks other than those stated in this description.      Qualifications
