Summary:
We are seeking a compassionate and dedicated Public Health Nurse to join our team. This role focuses on promoting community health, preventing disease, and providing outreach, education, and clinical services to individuals and families. The ideal candidate is a strong communicator, effective problem solver, and committed to improving public health outcomes.

Key Responsibilities:

  • Deliver nursing services including assessments, screenings, immunizations, and health education.
  • Participate in communicable disease investigations, case management, and reporting.
  • Provide community outreach and home visits as needed.
  • Support public health programs such as maternal and child health, emergency preparedness, and chronic disease prevention.
  • Maintain accurate records, documentation, and compliance with public health regulations.
  • Collaborate with community partners, healthcare providers, and social service agencies.

Qualifications:

  • Current Registered Nurse (RN) license in the state of Colorado.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Experience in public health, community health, or related nursing field is highly desirable.
  • Strong communication, organizational, and documentation skills.
  • Ability to work independently and collaboratively in diverse community settings.
  • Valid driver's license and ability to travel for outreach activities.
