JOB DESCRIPTION AND POSITION REQUIREMENTS

JOB SUMMARY:
The Complex Discharge Coordinator performs RN Care Coordinator duties and has additional responsibilities. The Complex Discharge Coordinator is responsible for leading and coordinating the discharge planning process for high-risk, medically complex patients in the acute care setting. The role ensures safe, timely, and appropriate transitions of care by collaborating with the interdisciplinary team, patients, families, and post-acute providers. The RN applies clinical expertise, critical thinking, and resource management to resolve discharge barriers, solve complex problems, reduce length of stay, prevent readmissions, and support optimal patient outcomes.
 
MINIMUM QUALIFICATIONS:

  • Graduate of accredited school of Nursing; Baccalaureate degree preferred
  • Current RN licensure by the Louisiana State Board of Nursing
  • 3-5 years of hospital adult acute care nursing experience preferred
  • Current BLS certification through any International Liaison Committee on Resuscitation (ILCOR) provided courses such as but not limited to:  American Heart Association and American Red Cross.  The ILCOR course must include a manikin check off.  
  • Certification (CCM, CMAC, or ACM) in case management required or must be obtained within two (2) years of hire
  • Strong clinical judgement and critical thinking skills
  • Advanced care coordination and discharge planning expertise
  • Knowledge of post-acute care services and payer requirements
  • Basic understanding of Medicare, Medicaid, and Managed Care regulations
  • Effective written and verbal communication skills. Ability to communicate with patients and families in a manner that de-escalates conflict, respects individual values and goals, and facilitates shared decision making
  • Organized and efficient in establishing priorities and performing tasks within specific timeframes
  • Self-motivated and able to solve complex problems with minimal supervision
  • Views problems as a challenge and investigates alternatives which demand creativity, flexibility, and ability to work under pressure
  • Responsible for maintaining strict confidentiality of all verbal and written information and documents
  • Familiarity with and appropriate use of community resources

 
PHYSICAL DEMANDS:
Must possess good physical health.  Some requirements include but are not limited to standing, sitting or walking for long periods of time.  Lifting at least 10 pounds is required.  Must be able to work with a moderate level of noise.
