Client Care & Community Liaison Coordinator


Are you a compassionate, organized professional who enjoys helping families, building relationships, and creating an exceptional client experience?


We are seeking a Client Care Intake & Experience Coordinator to guide prospective clients and their families from the initial inquiry through intake, admission, service startup, and ongoing follow-up. This position serves as a trusted point of contact while coordinating with scheduling, recruiting, and leadership to ensure each client receives responsive, personalized support.


When client intakes are not being conducted or scheduled, this position will meet with referral sources and community partners, conduct outreach, and strengthen relationships that support agency growth.


Key Responsibilities

  • Respond promptly and professionally to client care inquiries and referrals.
  • Conduct structured in-home or virtual client intake meetings, and non-medical assessments.
  • Identify each client’s needs, goals, routines, preferences, home environment, desired schedule, and service expectations.
  • Explain services, pay sources, schedules, payment expectations, and the admission process.
  • Determine whether requested services are within the agency’s scope and current capacity.
  • Complete admission process which includes home visit. 
  • Develop individualized non-medical service plans and caregiver briefings.
  • Coordinate service start dates, caregiver requirements, schedules, client introductions, and other details with the operations team.
  • Follow up after services begin to measure satisfaction and address questions or concerns.
  • Document and resolve client and family concerns with compassion, professionalism, and urgency.
  • Track referral activity, admissions, conversion rates, time to service start, client satisfaction, and reasons referrals do not convert.
  • Meet with healthcare referral sources and community partners when no client intakes are being conducted or scheduled.
  • Represent the agency at community meetings, networking events, presentations, and outreach activities.
  • Collaborate with internal teams to improve the intake process and overall client experience.

Qualifications

  • Bachelor’s degree in business, marketing, healthcare administration, communications, or a related field, or an equivalent combination of relevant education and professional experience.
  • Experience in home care, senior services, healthcare intake or admissions, customer experience, community engagement, hospitality, or a related service field preferred.
  • Excellent interpersonal, verbal, and written communication skills.
  • Strong interest in community engagement and referral relationship development.
  • Self-starter who is creative, resourceful, and forward-thinking.
  • Strong analytical, data-driven, organizational, and time-management skills.
  • Ability to manage multiple referrals, client needs, deadlines, and follow-up activities.
  • Compassionate and professional approach to working with clients, families, caregivers, and healthcare professionals.
  • Reliable transportation and willingness to travel throughout the agency’s service area.
  • Proficiency with Google Workspace preferred.
  • Experience with electronic scheduling, client records, secure messaging, or customer relationship management systems is a plus.
