Care Coordinator – Home Care
Western CO -- Pay Range: $20-$25 an hour

PeopleCare Health Services is hiring a Care Coordinator to manage client caseloads and coordinate quality home care services across various counties in Western Colorado. This role includes assessments, client follow-up, caregiver coordination, scheduling support, documentation, and communication with families, providers, and partner agencies to ensure care is delivered effectively and compassionately.


What You’ll Do

  • Manage an assigned caseload of clients
  • Coordinate care with clients, families, caregivers, and community partners
  • Complete intake assessments and required client visits
  • Support caregiver scheduling and coverage needs
  • Help address client or staff concerns promptly and professionally


What We’re Looking For

  • Healthcare, case management, or caregiving experience preferred
  • Strong communication, organization, and problem-solving skills
  • Comfortable working independently, in a fast-paced environment, and as part of a team


Work Setting
This role includes working from home, attending occasional in-office meetings, visiting client homes, and traveling throughout the field. Travel is required up to 50% of the time.
