Home Care Quality Intern

Masonicare Home Health & Hospice - Cromwell, CT area


Day Shift / 40hrs


Essential Duties and Responsibilities

 

Quality and Performance Improvement 

  1. Assist with quality improvement projects related to HHCAHPS, rehospitalization rates, and 

 overall quality performance metrics

  1. Support initiatives aligned with CMS Home Health Quality Measures and agency quality 

 goals

  1. Collect, organize, and analyze patient experience and clinical performance data
  2. Identify trends and opportunities to improve patient outcomes and satisfaction
  3.  Assist in developing reports, dashboards, and educational materials
  4. Collaborate with interdisciplinary teams to support performance improvement efforts

Intake & Marketing 

  1. Utilize HealthPivots and other data tools to identify and target potential referral sources
  2. Analyze market data to support growth and outreach strategies
  3. Assist in developing and strengthening relationships with referral sources (e.g., hospitals, skilled nursing facilities; Independent Living /Assisted Living communities; physicians, and community partners)
  4. Support Intake operations including referral tracking, data entry, and documentation
  5. Assist with outreach initiatives, marketing materials, and communication efforts
  6. Contribute to projects aimed at improving referral conversion and intake efficiency
  7. Other duties as assigned.



Minimum Qualifications:


Education: Current enrollment in a Nursing, Health Administration, Public Health, or related healthcare degree program at an accredited university 

Experience: Prior internship, work, volunteer, or healthcare experience preferred, 

 but not required. Microsoft Office experience preferred.
