About the Role:
Talin Home Healthcare LLC is looking for a compassionate and dedicated Home Health Nurse to serve patients in the Culpepper area. Responsibilities:
- Provide skilled nursing care to home health patients in the Culpepper service area
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications, wound care, IV therapy, and other clinical treatments as ordered
- Monitor and document patient progress, vital signs, and changes in condition
- Coordinate care with physicians, therapists, and other healthcare team members
- Educate patients and family members on disease management, medications, and home safety
- Maintain accurate and timely clinical documentation in compliance with regulatory standards
Requirements:
- Active RN license in the state of Virginia
- Previous home health, community health, or clinical nursing experience preferred
- Strong assessment, critical thinking, and clinical decision-making skills
- Excellent communication and patient education abilities
- Reliable transportation and valid driver's license for travel within the Culpepper area
- Ability to work independently and manage a patient caseload efficiently
- Compassionate, patient-centered approach to care
- Must be willing to work at least 2 days a week with 1 of those days be either Monday or Tuesday.
