Alcott, a leading nonprofit agency dedicated to providing vital mental health and supportive housing services to low- and no-income adults across Los Angeles County, is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our DHS team.

Schedule: Wednesday through Friday, 7:00 a.m. to 7:30 p.m.
Hours: Full-Time, 36 hours per week
Salary Range: $35.00–$37.00 per hour, DOE

What You'll Do: The Impact You'll Make

As an LVN, you will provide essential nursing care and coordinate medical services for adults experiencing complex medical, mental health, substance use, and housing-related challenges. Working alongside an interdisciplinary team, you will help clients access care, manage medications, and navigate the healthcare system.

Medical Care & Coordination

  • Provide hands-on care: Deliver basic nursing services, including injections, medication administration, and ongoing monitoring within the LVN scope of practice.
  • Coordinate care: Schedule primary care and specialty medical appointments and communicate with providers to support continuity of care.
  • Support medication management: Maintain medication logs with the Program Manager for clients receiving psychiatry services from The People Concern and coordinate with pharmacies to ensure timely delivery of client medications.
  • Educate and encourage: Provide patient education and supportive services to help clients follow their prescribed medication regimens.

Collaboration & Care Integration

  • Work as one team: Collaborate with an interdisciplinary team to deliver comprehensive, integrated care for clients with co-occurring medical, mental health, and substance use needs.
  • Connect services: Communicate effectively with internal and external providers to promote continuity of care and reduce service gaps.
  • Respond in critical moments: Provide crisis intervention and triage clients to emergency services, including police, paramedics, or the fire department, when necessary.
  • Stay engaged: Participate in weekly staff meetings and individual supervision sessions.

Documentation & Compliance

  • Document in real time: Record all services accurately and promptly in the designated electronic health record or charting system.
  • Help clients navigate care: Maintain working knowledge of Medi-Cal benefits and assist clients in accessing appropriate healthcare services.
  • Protect confidentiality: Comply with HIPAA requirements and all agency confidentiality standards.
  • Support program operations: Perform other duties as assigned by the Director.

What You'll Bring: Your Qualifications

Licensure & Experience

  • A current, active California Licensed Vocational Nurse license in good standing is required.
  • One to three (1–3) years of clinical experience working with individuals with serious mental illness and/or other complex needs is preferred.
  • Experience in health sciences, social services, mental health, or a related setting is preferred.
  • Current CPR certification is preferred.
  • Military-experienced candidates are strongly encouraged to apply.

Skills and Abilities

  • Client-centered care: Ability to work effectively and compassionately with adults experiencing homelessness, mental health conditions, substance use disorders, and other complex needs.
  • Sound judgment under pressure: Ability to remain effective during emotional stress, conflict, crisis situations, or interactions with uncooperative clients.
  • Technology readiness: Familiarity with electronic health records and other workplace technology tools.
  • Confidentiality: Strong understanding of HIPAA, privacy, and nondisclosure requirements.
  • Driving requirements: Valid California driver’s license, clean driving record, and insurability under Alcott’s policy are required.
  • Pre-employment clearance: Successful Live Scan clearance is required.
