Field RN Case Manager

Daily Responsibilities of a Field RN Case Manager:

• Potential for face-to-face assessments in member home within (1^st choice) Brenham, College Station Bryan, Cuero (2^nd choice) Killen, Temple, Cameron or Waco
• Complete all documentation in the electronic health record; referrals for items/services, coordination of care
• Following up on members in the upgrade process to implement services
• Making outbound phone calls to members and providers to coordinate services

Schedule: Mon-Fri 8-5 (may work after 5PM to complete documentation)
Pay: 38-43/hr (weekly pay)
Length of Assignment: 6 months, potential to extend

Requirements of a Field RN Case Manager:

• Clear and active Texas RN license required
• Case management experience
• 2+ years of clinical nursing experience
• Remote experience preferred
• Ability to use Outlook, Skype, Excel, and Zoom needed.
• Reliable transportation, clear driving record, and insurance
• Computer literacy, Excel, Outlook, Word
