JOB SUMMARY
This position works under the general supervision of a physician, advanced practice provider, and/or nursing leadership to provide experienced para-professional nursing skills in treatment, care and safety of patients. Works within their scope of practice as defined by the Board of Nurse Examiners for the State of Texas.

EXPERIENCE AND EDUCATION

Required

  • Education
    Graduate of an accredited Licensed Vocational Nurse program from approved school of nursing. 
     
  • Experience
    1-2 years of post-high school education. 
     
  • Licenses and Certifications
    (LVN) LICENSED VOCATIONAL NURSE by Board of Nurse Examiners for State of Texas 
    (BLS) BASIC LIFE SUPPORT 
    (DRIVERS) DRIVERS LICENSE TX Driver's License and proof of driver's insurance

  • Experience
    2-3 years' experience in Home Health Care
     


JOB DUTIES

  • Provides direct nursing care, including patient observation and delivery of prescribed treatments.
  • Monitors medical condition of patients and reports changes to appropriate personnel.
  • Maintains patient records and charts to ensure an accurate record of treatment.
  • Provides instructions to patients/families for use of products and services utilizing nursing process to address learning needs.
  • Prepares, hangs, monitors, and discontinues IV fluids. Accesses IV delivery system; i.e., peripheral IV start, implant cath, vascular access devices, etc.
  • Responsible for overall delivery of quality patient care that meets or exceeds established standards and provides assistance in activities to meet state and/or federal regulatory guidelines.
  • Maintains a safe, comfortable, and therapeutic environment for patients/families in accordance with established standards.
  • Participates in Organizational Quality Improvement Program (QAPI).
  • Participates in in services and staff meetings.
  • Duties performed may include one or more of the following core functions: (a) Directly interacting with or caring for patients; (b) Directly interacting with or caring for human-subjects research participants; (c) Regularly maintaining, modifying, releasing or similarly affecting patient records (including patient financial records); or (d) Regularly maintaining, modifying, releasing or similarly affecting human-subjects research records.
  • Performs other duties as assigned.
