Job Description:

JOB DESCRIPTION

GENERAL DESCRIPTION:

The ePEP Case Manager-QMHP will function as an intensive case manager and health coach to patients with first episode of psychosis on the CSC Team Services may include, but are not limited to, providing health coaching, skills training, treatment, assisting with communication to the providers & CSC Team, monitoring symptoms & side effects, working closely with CSC Family Peer Support Specialists on a recovery plan, co-facilitating psychoeducational groups, advocating for the patient, crisis intervention, providing continuity of care, monitoring medication use, and participation in treatment. The overall goal of this position is to assist patients on the CSC team to reach their maximum potential.

This position is travel based throughout Dallas County

Starting Pay $24.04 - review dependent on experience

ESSENTIAL DUTIES AND RESPONSIBILITIES:


Facilitate psychoeducational and recovery groups using a specific curriculum.
Provide overall service coordination, referral, linkage, and advocacy to patients with varying needs. Perform duties in concert with other members of CSC Team.
Participate in CSC team meetings.
Follow up assigned patients who missed a scheduled appointment. Document attempts to contact consumer.
Follow up with assigned patients within 24 hours on assigned patients who access emergency services. Report status and disposition to treatment team.
Provide crisis intervention. Make home visits, initiate mental illness warrants as appropriate, work with police and other public servants as a needed to address crises. Includes on-call services.
Assist with inpatient admission when applicable. Provide continuity of services throughout inpatient stay. Participate actively in discharge planning.
Perform other duties as assigned.

QUALIFICATIONS

EDUCATION AND EXPERIENCE:


Masters degree from an accredited college or university in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention.


COMPUTER SKILLS:


Use computer, printer, and software programs necessary to the position (e.g., Word, Excel, Outlook, PowerPoint)
Ability to utilize Internet for resources.

While performing the duties of this job, the incumbent is regularly required to talk and hear, use hands and fingers to operate a computer and telephone. Due to the multi-site responsibilities of this position the incumbent must be able to carry equipment and supplies (up to 15 pounds). The position requires standing and/or walking in excess of 7 hours a day.

TRAVEL:


In-county travel.
No overnight travel required.

Positions that are community-based, in whole or part, require the incumbent to travel between various work sites within his/her workday/workweek. The incumbent is required to have a valid drivers license and reliable transportation. Failure to meet these terms may result in disciplinary action up to and including termination of employment, contract or other status with Metrocare.
