Job Description Summary: The Licensed Practical Nurse (LPN) is a nursing care provider who functions under the direct supervision of the Residential Administrator. The LPN provides nursing duties assigned and within the scope of nursing license.  The LPN provides services to individuals impacted by a variety of behavioral health issues, recognizing that the individuals we serve often have experienced trauma that affects their development and adjustment. The LPN demonstrates a warm and welcoming empathic, hopeful attitude, conveying a philosophy of dual recovery.  The LPN will be trained in trauma-informed care and will work to provide services in an environment that is sensitive and responsive which will prevent victimization, abuse, or trauma as a result of the care received. The LPN reports to the Residential Administrator. 

Education:  

  • High School Diploma or equivalent.   

  • Diploma, degree, or certification in Nursing 

Preferred Qualifications:

Experience in nursing in behavioral health is preferred. 

 

Responsibilities:  

  • Meets weekly with each residential client and thoroughly documents the assessed care needs inmonthly (at minimum) nursing notes.   

  • Consults and coordinates care with each client’s care providers, including psychiatric, primary care, OC Healthcare Home, and the clinical team members. 

  • Serves as lead in the medication room, ordering and checking in medications and relaying pertinent information to staff responsible for medication administration.  Any delegation of this duty must be approved by the Residential Administrator. 

  • Prepares medications for clients who are scheduled for leave of absence from the facility for more than a 24-hour period and when multiple doses of medications are required. 

  • Provides TB testing to facility residents and Ozark Center staff.    

  • Administers medications and injections accurately as prescribed. Orders and follows up on labs as prescribed by the physician. Reviews all Medication Administration Records weekly to ensure that physician orders and medication administration records are accurate and current. 

  • ProvidesLevel 1 Med Aide training, including insulin administration, for certification and recertification of staff. 

  • Completes all required documentation within established timeframes and according to quality standards set by State and Federal regulations, CARF certifications, and program expectations.   

  • Performs all other duties as assigned

Physical Requirements:  

  • Typical demands include prolonged sitting, frequent standing, bending, stooping, and occasional lifting.  

  • Requires range of hearing and manual dexterity sufficient to operate keyboard, telephone, photocopier, calculator, and other office equipment as needed.   

  • Requires ability to work under stressful conditions, includingirregular hours and manage difficult behaviors. 

  • Requires use of a computer   

  • TB test upon hire.
