Renewed Horizons is seeking an organized, compassionate, and motivated Supported Employment Case Manager to join our team. This position plays an important role in helping individuals with disabilities achieve meaningful employment, build workplace skills, and increase independence.



The Supported Employment Case Manager oversees participant services from planning through implementation. This includes developing individualized employment plans, monitoring progress and data, attending person-centered planning meetings, communicating with participants and their support teams, and ensuring services remain meaningful, person-centered, and aligned with each participant’s goals.



This is a hands-on position. In addition to case management responsibilities, the Case Manager will provide job coaching when needed and may assist participants with job development, employer outreach, applications, interviews, and other employment-related activities.



KEY RESPONSIBILITIES

  • Develop and update individualized employment plans and workplace goals.
  • Write clear, measurable objectives based on each participant’s strengths, needs, preferences, and employment goals.
  • Review job coaching documentation and track participant progress and service data.
  • Monitor authorized service hours and ensure services are provided appropriately.
  • Attend person-centered planning meetings and other participant team meetings.
  • Prepare progress reports, assessments, and other required documentation.
  • Communicate regularly with participants, families, guardians, support coordinators, employers, job coaches, and other members of the participant’s team.
  • Observe participants at their workplaces to evaluate progress and identify support or training needs.
  • Provide job coaching and workplace support when needed.
  • Help train and guide job coaches regarding participant plans, teaching strategies, prompting, documentation, and fading supports.
  • Assist with job development, employer communication, job applications, interviews, and workplace onboarding as assigned.
  • Promote participant choice, dignity, independence, and inclusion in all services.
  • Maintain accurate, timely, and confidential records.
  • Follow Medicaid, Idaho Department of Health and Welfare, agency, and program requirements.



WHAT WE ARE LOOKING FOR

  • Strong written communication and documentation skills.
  • Excellent organization, time management, and follow-through.
  • Ability to write measurable, person-centered plans and goals.
  • Ability to review data and use it to guide services.
  • Confidence communicating with participants, families, employers, and professional team members.
  • Ability to balance office-based case management with hands-on community and workplace support.
  • A genuine belief that individuals with disabilities are capable of growth and meaningful employment.
  • Experience in supported employment, developmental disability services, vocational rehabilitation, case management, social services, education, or a related field is preferred.
  • Job development or employer-relations experience is helpful but not required.
  • Must meet agency requirements for driving, background checks, training, and service delivery.
