 Schedule: Primarily Monday–Friday, with flexibility based on participant work schedules and meetings

Are you organized, compassionate, and passionate about helping people succeed? Renewed Horizons is seeking a Supported Employment Case Manager to coordinate employment services for adults with developmental disabilities. This position combines individualized planning, progress monitoring, team collaboration, and hands-on workplace support to help each participant grow in confidence and independence.

What You’ll Do:

    ✓ Write and update individualized employment plans and measurable workplace goals

    ✓ Review job coaching documentation and track participant progress and service data

    ✓ Prepare progress reports, assessments, and other required documentation

    ✓ Attend person-centered planning (PCP) meetings and other participant team meetings

    ✓ Communicate with participants, families, support coordinators, employers, and job coaches

    ✓ Observe participants at work and help guide job coaches in implementing plans consistently

    ✓ Provide job coaching and workplace support when needed

    ✓ Assist with job development, applications, interviews, and employer outreach as assigned

What We’re Looking For:

    ✓ Strong written communication, organization, and time-management skills

    ✓ Ability to write clear, observable, and measurable goals

    ✓Ability to review data and use it to guide participant services

    ✓ Experience in supported employment, developmental disability services, case management, education,

        vocational services, or a related field is preferred

    ✓ Job coaching or job development experience is helpful

    ✓ Must be at least 18 years old

    ✓ Valid driver’s license, appropriate car insurance, and reliable transportation

    ✓ Must pass a federal background check

    ✓ CPR/First Aid certification required—don’t have it yet? We can provide the training
