Summary: The Service Manager 3 will provide support and assistance in Level 3 day habilitation programs. Staff will support individuals who need daily intervention, supervision, and skills training in activities of daily living within a day program environment and in the community. Individuals may require moderate to considerable physical assistance or accommodation. Staff will receive specialized training to provide enhanced supervision or interventions designed to address multiple disabilities which may include behavioral supports. Staff will develop, implement, and supervise a training program designed to meet each individual's needs, case management of designated individuals, or participation in general duties.

Schedule: Monday - Friday 8:00am - 4:00pm (40 Hours)

Essential Duties and Responsibilities:

  1. Provide services to individuals according to DDS, DMH, MassHealth, HUD, DPH, CARF and all applicable regulatory standards as well as Bridgewell's policies and procedures.
  2. Administer and interpret assessments to determine each individual's skill level on an annual basis.
  3. Plan and implement an individualized program, based on each individual's needs following their Day Habilitation Support Plan.
  4. Teach skills to individuals and small groups for training in designated area.
  5. Develop adaptive equipment and jigs to ensure individuals attain goals.
  6. Supervise the implementation of the Day Habilitation Support Plan and maintain the individual records in TIER.
  7. Assist people in making decisions about their daily routine/activities.
  8. Assure that people have access to communication with family, friends, and significant others.
  9. Assist people in participating in community events including volunteer sites, libraries, YMCA's and any particular place of interest to them.
  10. Maintain appropriate certifications including FA/CPR and PABC
  11. Other Duties as assigned.

Required Education/Experience:

  1. Minimum of Associates Degree in Human Services or equivalent experience.
  2. A minimum of one year of experience working with people with developmental or psychiatric disabilities required.
  3. Training and/or experience in areas such as autism and/or behavioral support.

Required Skills/Knowledge:

  1. Knowledge and understanding of principles of normalization, behavior management, and human rights
  2. Excellent written and oral communication skills so that the writing of required documents is clear and articulate.
  3. Excellent organizational and problem-solving skills.
  4. Valid driver's license and one year of driving experience.
  5. Computer literacy to include ability to proficiently use Microsoft products and web based applications supported by the agency.

Physical Demands:

  1. While performing the duties of this job, the employee is frequently required to climb stairs.
  2. Must be able to lift up to 50 pounds.
  3. The employee must be able to drive a vehicle and regularly meet driving record requirements of the agency.
  4. Must have a normal range of eyesight with or without corrective lenses.
  5. Must have a normal range of hearing with or without corrective equipment.
