Job Summary:

Coordinates patient care and maximizes patient satisfaction and experience. Partners with care providers to serve as a liaison to ensure effective communication with patients.

Job Duties:

  • Provides discharge information.
  • Partners with providers to ensure a smooth transition at discharge to home or other facilities as required.
  • Ensures additional resources are made available for the patient as needed.
  • Effectively collaborates with other healthcare providers and non-Geisinger employees in coordinating and implementing the patients plan of care.
  • Maintains systems and processes to assure timely follow-up and surveillance of a defined group of patients.
  • Serves as liaison with clinical and administrative personnel.
  • Establishes point of contact for referring providers to create immediate access to provider templates.
  • Develops and implements efficient patient navigational procedures.
  • Investigates patient concerns including billing, appointment access, perception of care, physician issues and communication problems.
  • Develops, maintains, and audits standard operating procedures of patient access and utilization process.
  • May provide administrative support as needed.
