Social Worker - Adults

Job Title: Social Worker - Generic 
Location: Darlington
Pay Rate: £26 per hour

Liquid Personnel is recruiting an Adult Social Worker to join its client’s Generic team, based in Darlington.

What will your responsibilities be? 

• Facilitate safe and timely hospital discharges using intermediate care or long‑term support services, ensuring a smooth “home first” transition.
• Complete person‑centred, strengths‑based assessments and ongoing reviews to support recovery and determine longer‑term care needs.
• Work both independently and within the RIACT multidisciplinary team while maintaining high‑quality case recording and applying the Care Act 2014 and discharge procedures.

Qualifications and Experience: 

To be successful in this role you must have;  

• Social Work England registration 
• Eligible to work in the UK 
• Hold a full UK driving licence 
• Post-qualified experience 
• Degree level or equivalent in Social Work
