Job Title: Social Worker - GenericĀ

Pay Rate: £26 per hour
Liquid Personnel is recruiting anĀ Adult Social Worker to join its clientās Generic team, based in Darlington.
What will your responsibilities be?Ā
- Facilitate safe and timely hospital discharges using intermediate care or longāterm support services, ensuring a smooth āhome firstā transition.
- Complete personācentred, strengthsābased assessments and ongoing reviews to support recovery and determine longerāterm care needs.
- Work both independently and within the RIACT multidisciplinary team while maintaining highāquality case recording and applying the Care Act 2014 and discharge procedures.

Qualifications and Experience:Ā
To be successful in this role you must have; Ā
- Social Work England registrationĀ
- Eligible to work in the UKĀ
- Hold a full UK driving licenceĀ
- Post-qualified experienceĀ
- Degree level or equivalent in Social WorkĀ
