The system quality APRN plays a crucial role in process improvement. To fill this role, they must maintain the skills necessary to assess, diagnose, and manage hospitalized patients. These skills then help them work within a multidisciplinary team, collaborating with physicians, nurses, and other healthcare professionals to provide comprehensive care focused on improving outcomes related to a specific patient population. The System Diabetes APRN will partner with the physician leaders and other related service line and clinical program leaders to ensure a comprehensive, market competitive, and leading-edge clinical program at affiliated sites across the system and to outside referral entities. The program APRN is responsible for tactical execution and management of the Inpatient Diabetes Program by coordinating the overall program, ensuring processes are in place to provide seamless transition of care inpatient and outpatient environments, act as a liaison clinical practice, hospital administration, and senior system leadership The program APRN will be responsible for monitoring, interpreting, & disseminating quality performance data and in turn using that data to lead process improvement related to diabetes for the system.
The (APRN) is a practitioner, educator, consultant, and researcher who collaborates with an inter-disciplinary team and advocates optimal experience for patients/families. He or she provides physical, emotional and spiritual support which impacts patient outcomes in a safe environment. The APRN functions in a versatile role that demonstrates professionalism and promotes excellence and autonomy in the practice of Nursing. The APRN has in–depth knowledge of evidence-based practice within a specialty that results in competencies to expand the boundaries of nursing practice by, (a) addressing illness and promoting population health, (b) advancing the practice of other nurses and (c) developing organizational/system modifications to support and improve the practice of nursing.
This clinical expert is committed to evidence-based practice, professional standards of care, and code of ethics. Acts as a consultant to the patient, family, healthcare organization and community to assist patients to attain their maximum level of health in a manner sensitive to social and cultural diversity. The role of an APRN may vary based upon qualifications through licensure, accreditation, certification, education and job expectations. APRN practice is facilitated through the standard care agreement between the APRN and collaborating physician(s). Many APRN’s participate in clinical decision making, care plan development and prescribing practices. The age of population served is defined by their specific Advance Practice Provider license.

Minimum Level of Education Required: Master's degree in nursing as a  Adult CNS or NPMaster's degree

Licensure/Certification/Registration
§ License to practice as a registered nurse in the State of Ohio
§ Valid license to practice as an Advanced Practice Registered Nurse in the State of Ohio
§ Current CPR certification required at the time of hire
§ National Certification as an Advance Practice Nurse or active Certificate if Authority
Experience

Knowledge/Skills
- Two references from APRN’s in the clinical or educational settings
- Reference of collaborating physician IF they have been in this role
- PHP Standard Care Arrangement prior to engaging in practice
- Must be able to multitask with constant interruptions while maintaining a pleasant demeanor
- Basic computer knowledge
- Strong organization skills
- Effective interpersonal and communication skills
