Position Overview

We are seeking a passionate and patient-centered Physical Therapist (PT) to join our growing team. In this role, you will deliver high-quality therapy services to residents in assisted living environments, helping them restore mobility, function, and confidence.

Key Responsibilities

  • Provide treatment visits, 30-day reassessments, re-certifications, and discharge evaluations
  • Develop and implement individualized treatment plans with measurable short- and long-term goals
  • Supervise and mentor Physical Therapist Assistants (PTAs)
  • Manage a full caseload (28–32 visits per week for full-time positions)
  • Maintain accurate documentation using an electronic medical record system
  • Recommend and assist in fabrication of adaptive devices as needed
  • Educate patients, families, and caregivers on therapy techniques and equipment use
  • Collaborate with interdisciplinary healthcare teams to ensure coordinated, high-quality care

Qualifications

  • Degree in Physical Therapy from an accredited program
  • Active state Physical Therapist license
  • CPR/BLS certification
  • Valid driver’s license and auto insurance
  • Strong knowledge of medical terminology and therapy best practices
  • Excellent communication and interpersonal skills
  • Compassionate, empathetic approach to patient care

Preferred:

  • Geriatric experience
  • Experience with care planning, discharge planning, or inpatient settings
