JOB SUMMARY:

Creates linkages/referrals to local public and private service providers to empower families to make progress toward achieving economic independence and housing self-sufficiency; or for elderly or disabled residents, help improve living conditions and enable residents to live/age-in-place.

ESSENTIAL JOB FUNCTIONS:

Under the general supervision of the Resident Services Manager:

  1. Provides case management services to residents, including:
    1. Making appointments with current and prospective clients;
    2. Evaluating and assessing client’s current situation and needs;
    3. Making determination of need for additional services, education and training;
    4. Making referrals to other agencies that can provide needed assistance and training; and
    5. Coordinating overall services provided to residents to enable them to become self-sufficient.
  2. Meets with residents to assess needed service levels, provide training as needed and evaluate progress.
  3. Coordinates other transportation as needed to enable clients to attend meetings with social service agencies and providers, training, job interviews and visit and evaluate other potential housing.
  4. Meets with social service agencies to coordinate appropriate service provision to clients to enable them to become self-sufficient.
  5. Provides and coordinates training to clients and prospective clients through individual contact and community meetings as required or requested.
  6. Completes documentation of client contacts in an accurate and timely manner according to Authority guidelines.
  7. Sponsor educational events that include subjects relating to literacy, computer technology, healthcare, agency support, life skills referral sources, and others.
  8. Develop a Resource Directory that includes a listing of state and/or local service providers.
  9. Facilitate the formation of resident councils within the property if there is interest amongst residents.
  10. Obtain Professional Service Coordinator (PSC) certification from American Association of Service Coordinators.
  11. Performers other tasks as directed by the Resident Services Manager.

SUPERVISORY RESPONSIBILITIES:

None.

WORK ENVIRONMENT:

Includes both a typical office environment, with minimal exposure to excessive noise or adverse environmental issues and outside local travel, including occasional exposure to heat, cold, inclement weather and environmental hazards such as dust, fumes, mold and vapors.

QUALIFICATIONS AND REQUIREMENTS:

The incumbent must:

  1. Have a minimum of an Associate’s degree or one to three years’ related work experience.
  2. Hold a valid Ohio driver’s license.
  3. Have proficiency in Microsoft Office applications.
  4. Be able to communicate clearly and precisely, both orally and in writing.
  5. Be able to meet the following physical requirements with or without reasonable accommodation:
    1. Walk, stand and sit;
    2. Use hands to manipulate, handle, feel and control items or equipment;
    3. Talk and hear;
    4. See and be able to read, write and interpret text, spreadsheet, and graphical documents;
    5. Occasionally lift and move objects weighing up to 25 pounds.
