About Heritage Behavioral Health Center

Heritage Behavioral Health Center is a mission-driven Certified Community Behavioral Health Clinic (CCBHC) serving individuals across Central Illinois. We are committed to improving access to high-quality mental health and substance use services and believe every team member plays a meaningful role in supporting the well-being of our community.

We are currently seeking a compassionate, community‑focused Telehealth Navigator to expand access to behavioral health services through technology, outreach, and personalized support.

Position Overview

The Telehealth Navigator plays a vital role in helping individuals involved in community support programs access and fully participate in telehealth services. This position works directly within the community to assist clients with completing their Individualized Assessment and Treatment Plans (IATP) in collaboration with clinicians via telehealth platforms.

This is a hands-on, mobile role that supports clients where they are—removing barriers, promoting engagement, and empowering individuals to achieve their wellness goals.

Core Responsibilities

Direct Client Support

  • Serve as a primary point of contact for individuals participating in community support programs.
  • Assist clients in completing IATPs through scheduled telehealth sessions with clinicians.
  • Provide step-by-step education on telehealth technology, ensuring clients can confidently participate in virtual appointments.

Community-Based Outreach & Navigation

  • Conduct outreach throughout the community to identify individuals needing telehealth support.
  • Build strong relationships with local agencies, service providers, and community partners.
  • Advocate for clients and connect them with additional resources and support services as needed.

Collaboration & Care Coordination

  • Work closely with clinical teams to ensure continuity of care and effective communication between clients and providers.
  • Maintain accurate documentation of client interactions, progress, and identified needs.
  • Stay informed on emerging best practices in telehealth delivery and community support services.

Knowledge, Skills, and Abilities

  • Ability to work collaboratively across multiple programs and multidisciplinary teams
  • Comfort working in environments involving ambiguity, uncertainty, and change
  • Ability to navigate electronic health records (EHR)
  • Strong organizational skills with the ability to manage your own schedule
  • Knowledge of serious mental illness (SMI) and co-occurring chronic health conditions
  • Ability to lift up to 15 pounds on a regular basis
  • Strong communication and interpersonal skills
  • Commitment to supporting individuals in a respectful, empowering manner

Education & Requirements

  • High school diploma or equivalent (required)
  • Minimum 21 years of age
  • Valid driver’s license, reliable transportation, and proof of automobile insurance (required)
