Job Description:

Position Summary:

   

The Weekend Clinical Care Coordinator, is responsible for monitoring and responding to referrals through Allscripts, E-fax and 4 Next and data entering referral information provided in the EMR, answering incoming telephone calls from patients/patients' families and referral sources; making patient care decisions in response to telephone calls; coordinating and assigning visits to clinicians as deemed medically necessary to meet patients' needs; and accepting reports from and responding to questions from home care clinicians who are in the field providing care.

Position Responsibilities Include:

  • Respond to incoming telephone calls and other inquiries from patient/patients' families, referral sources and other healthcare professionals.

  • Document incoming patient telephone calls, response or direction, and the coordination of services using the appropriate processes and systems in the EMR

  • Make patient care decisions in response to incoming patient telephone calls, using professional nursing judgment. Such responses may include providing advice or direction to the patient/cg, contacting the physician or scheduling a visit.

  • Provide guidance and direction to home care providers regarding patient care and clinical issues including patient/staff incidents and/or injuries.

  • Regularly communicates with and works in cooperation with appropriate personnel to coordinate patient care services.

  • Oversees weekend schedules, and makes adjustments to schedule in response to patient and and/or staffing changes.

  • Perform the duties and responsibilities of the Intake Coordinator position, by monitoring and responding to all referrals through Allscripts, E-Fax and 4 Next, in addition to reviewing content of referrals and data entering referral information, and current orders into EMR.

  • Uses EPIC to gather information and DC paperwork as appropriate.

  • Coordinates and assigns admission visit to appropriate staff when weekend or Monday admission is indicated.

  • Check Common Working File for MC patients for Admissions as required by the payer as able, given potential limitations of accessibility during weekend days.

  • Check managed care to verify coverage for new referrals.

  • Will be responsible to receive hand off communication from Clinical Manager regarding any potential issues (patient related or staff related) that may possibly occur on the weekend.

  • Provide weekend summary report to intake department, Sr. Director of Home and Community-Based Services and Home Health and Hospice Clinical Manager for their review on Monday mornings.

  • As directed, collaborates to create an agency environment that is in compliance with federal, state, and local regulatory agencies.  Complies with HIPAA regulations and practice policies for the privacy and security of patient information.

  • Maintains compliance with policies, procedures, and regulatory matters.

Qualifications: 

  • Proficient computer skills.
  • Experience with documenting in EMR 
  • Good verbal and written communication skills.
  • Must be motivated to learn and flexible to change.
  • Strong customer service skills.
  • Working knowledge of Medicare regulations for certified home care and hospice.
  • Prior Experience Intake/Office Nurse preferred
  • Current MA license to practice as a Licensed Practical Nurse (LPN)
  • 2 years of experience in Certified Home Care or Hospice as a nurse
