About the Role

Deerfield Supportive Living is seeking a compassionate and dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our clinical team. In this  position, you will play an essential role in providing high-quality, person-centered care while supporting the health, safety, and well-being of our residents. Working under the direction of the Wellness Director, you will collaborate with fellow team members to deliver exceptional clinical care in a warm and supportive living environment.

Primary Responsibilities

  • Provide daily nursing care and clinical services in accordance with individualized care plans.

  • Perform resident assessments, monitor vital signs, and promptly report changes in condition.

  • Administer medications accurately and maintain complete and timely documentation.

  • Collaborate with Certified Nursing Assistants (CNAs) and interdisciplinary team members to support resident care.

  • Provide clinical leadership, guidance, and support to direct care staff.

  • Promote a safe, positive, and resident-centered environment.

  • Perform additional duties as assigned by the Executive Director or Wellness Director.

Minimum Qualifications

  • Current Illinois RN or LPN license in good standing.

  • Current CPR certification.

  • Strong clinical assessment, communication, and organizational skills.

  • Experience administering medications and maintaining accurate clinical documentation.

  • Ability to work independently while collaborating effectively with the care team.

Preferred Qualifications

  • Previous experience in supportive living, assisted living, senior living, skilled nursing, or long-term care.

  • Experience working with older adults and individuals with varying healthcare needs.

Physical Requirements

  • Ability to lift up to 25 pounds and assist residents with mobility and activities of daily living as needed.

  • Ability to stand, walk, bend, and move throughout the community during scheduled shifts.
