Albany Center for Independent Living is actively seeking a Medication Room Desk Nurse (LPN) to work for our Adult Home in Delmar, NY.

Duties:
  • Assist with daily medication ordering and refilling processes.
  • Ensure medications are accurately poured into individual cups and administered as prescribed.
  • Verify consistency between physician prescriptions, blister packs, and administered medications.
  • Review and manage medication refill logs, including handling "no-refill" orders and requesting renewals from physicians.
  • Maintain and update medical and medication records for all residents.
  • Review and document the receipt, storage, and handling of medications delivered.
  • Track and follow up on missed medications, ensuring proper documentation and communication with Case Management.
  • Follow protocols for the storage and destruction of controlled and non-controlled substances.
  • Ensure proper inventory management and safekeeping of controlled substances.
  • Assist in coordinating the destruction of controlled substances with the staff nurse and DOH.
  • Report medication issues and concerns to the Medication Supervisor and Administrator.
  • Communicate effectively with physicians regarding medication orders and lab results.
Requirements:
  • Valid NY State LPN license.
  • Clear communication skills in English, both written and verbal.
  • Proficiency in using word processing programs and basic computer functions.
  • Understanding of the needs of elderly and infirm patients.
  • Strong organizational skills with the ability to set goals and meet deadlines.
  • Ability to perform basic mathematical calculations related to medication dosage.
