Job Description:
OVERVIEW OF POSITION:
The Community Partnership Representative is responsible for driving patient growth through strategic community engagement, relationship development, and outreach initiatives focused on Medicare-eligible seniors. This role serves as a key liaison between the organization, community partners, health plans, brokers, referral sources, and prospective patients to increase awareness of value-based primary care services and support patient enrollment.
The Community Partnership Representative develops and maintains partnerships that generate referrals, coordinates and participates in community events, delivers educational presentations, and collaborates with internal teams to ensure seniors have access to preventive healthcare services. This position is a hybrid role, requiring approximately 50% field-based work and 50% home office responsibilities.
ESSENTIAL DUTIES:
- Develop and maintain relationships with referral sources, community organizations, health plans, brokers, and other strategic partners to support patient growth initiatives.
- Identify and cultivate new community partnerships that expand awareness of the organization's services and increase patient referrals.
- Deliver educational presentations and seminars to prospective and existing patients, community groups, and partner organizations.
- Attend and represent the organization at community events, health fairs, networking functions, and lead-generation activities.
- Engage prospective patients through phone, text, email, virtual, and in-person outreach efforts.
- Manage, track, and report lead activity, referral sources, and enrollment outcomes using CRM and reporting tools.
- Coordinate with health plan partners to support outreach initiatives, educational events, and patient engagement opportunities.
- Develop and maintain productive relationships with local insurance brokers and broker organizations.
- Partner with marketing to promote community events, support social media initiatives, and contribute to content development.
- Collaborate with clinic operations and care teams to coordinate patient tours, educational events, and community engagement activities.
- Build relationships with nonprofit organizations, senior-focused groups, community centers, and other community partners to generate referrals and new patient opportunities.
- Work closely with health plans, brokers, and community stakeholders to increase awareness of the organization's value-based care model.
- Support prospective patients through the enrollment process and facilitate connection to a primary care provider.
- Achieve patient growth and enrollment goals through consistent relationship building, outreach efforts, and community engagement activities.
- Maintain a professional presence in the community and serve as a trusted ambassador for the organization.
EDUCATION:
- Bachelor's degree in Business, Marketing, Communications, Healthcare Administration, Public Health, or a related field preferred.
- Equivalent combination of education and relevant work experience may be considered.

EXPERIENCE:
- Knowledge of Medicare products, healthcare services, and senior populations.
- Understanding of Value-Based Care principles and population health management preferred.
- Knowledge of community outreach, business development, and relationship management practices.
- Familiarity with referral networks, community organizations, and healthcare partnership development.
- Working knowledge of CRM systems, Microsoft Office applications, and reporting tools.
- Understanding of healthcare compliance and professional standards related to patient engagement activities.

- Exceptional interpersonal and relationship-building skills with the ability to establish credibility and trust with diverse stakeholders.

- Proficiency in Microsoft Excel, PowerPoint, Outlook, and CRM platforms.
- Ability to analyze outreach results and utilize data to improve performance.
- Demonstrated ability to work independently while collaborating effectively across multiple departments.
- Strong customer service orientation and commitment to creating positive experiences for patients and partners.
- Ability to adapt to changing priorities in a fast-paced, growth-oriented environment.
- Ability to travel locally and work occasional evenings or weekends to support community events and outreach activities.
Salary Range:
Salary Range: $90-120K base, with the opportunity to earn an additional $20,000-$40,000 in performance-based incentive compensation. Total compensation is tied to achievement of established performance objectives.
Additional Compensation: Eligible for annual bonus based on individual and/or company performance.
