Description
We are looking for a full-time Care Navigator to work Monday - Friday in our Denver clinic located at 1601 E 19th Ave Suite 6600, Denver, CO 80218. Every Child Pediatrics’ Care Navigation is a collaborative process directed toward patient and family wellness and autonomy through advocacy, assessment, planning, communication, education, resource management, and service facilitation. We provide patient- and family-centered care tailored to individual needs and values, working hand-in-hand with staff members and community partners. Care Navigators empower families to overcome barriers and access essential resources, such as financial assistance, translation, transportation, housing, food, and early intervention referrals, by guiding them through complex systems with knowledge and hands-on support.
Responsibilities
- Help families access and coordinate medical specialty care, community services, developmental assessments, and school planning
- Build trusting relationships with patients and families through phone calls, texts, and in-person meetings at the clinic, community spaces, schools, homes, or court
- Serve as the primary point of contact, advocate, and informational resource for patients/families and community partners/payers
- Work closely with healthcare providers within a team-based approach to care to identify patients with special and complex needs and initiate the care plan process
- Develop, organize, and maintain up-to-date, comprehensive, and culturally responsive resources for patients and families
- Maintain accurate, thorough, and timely records of all activities and provide data for reports
- Treat all clients with dignity and respect and honor their right to privacy
Requirements
- Bachelor’s degree in social work, psychology, case-management, advocacy, or a related field (or the equivalent healthcare experience with a focus on patient care, case management, resource coordination, public health, or health education)
- Will consider applicants with an associate degree with additional related experience, education and/or training in aligned areas
- Previous experience working in a medical setting, including the ability to work with medical professionals, review medical records, and apply health and wellness principles
- Bilingual in Spanish/English is strongly preferred (a $1,000 signing bonus will be provided to candidate who are bilingual)
- Excellent verbal and written communication skills
- Ability to analyze, solve problems, and demonstrate innovative thinking
