An RN Start of Care (SOC) nurse in home health completes the patient’s initial in-home admission and establishes the clinical plan for ongoing care.
Typical responsibilities include:
- Verify the patient’s identity, eligibility, homebound status, referral orders, and consent forms.
- Complete a comprehensive head-to-toe assessment, including vital signs, pain, cognition, mobility, skin integrity, nutrition, elimination, and psychosocial or safety needs.
- Review diagnoses, medications, allergies, recent hospitalization, and treatment history; identify discrepancies and notify the provider as needed.
- Complete required admission documentation, including the OASIS assessment when applicable.
- Evaluate the home environment, caregiver support, fall risk, emergency preparedness, and need for equipment or community resources.
- Develop an individualized plan of care with measurable goals, visit frequency, interventions, and discharge planning.
- Perform ordered skilled nursing services, such as wound care, medication management, injections, catheter care, or disease monitoring.
- Educate the patient and caregivers about diagnoses, medications, infection prevention, warning signs, emergency procedures, and when to contact the agency or call 911.
- Coordinate orders and services with physicians, therapists, social workers, aides, pharmacies, and medical-equipment providers.
- Document the visit accurately and promptly and communicate significant findings or changes in condition.
- Determine whether home-health admission is appropriate and escalate immediate safety or clinical concerns.
Requirements
Current RN License
Valid Driver’s License
CPR Certification
Valid Auto Insurance
At least 2 years of Home Health RN experience preferred
HomeCare HomeBase experience preferred
