The primary role of the Hematology/Oncology Pharmacist is to enhance the coordination of patient care across the healthcare continuum. The focus is on identifying medication-related interventions that will facilitate the delivery of high quality and cost-effective clinical outcomes in oncology and hematology patients.

Responsibilities

Clinical Excellence/Patient Care

  • Coordinate chemotherapy-related medication order entry and administration between the clinic, infusion center, hospital, home health, and retail pharmacies by communicating with interdisciplinary members of the healthcare team.
  • Complete comprehensive chart review of patients’ allergies, immunizations, and medications to assess appropriateness of regimen, identify and mitigate adverse drug events (ADE’s), evaluate and provide action plan for managing medication interactions, and promote cost-effective medications and services.
  • Assist in all oncology medication therapy management activities including but not limited to dosing and monitoring recommendations, prophylaxis supportive cares, and/or anticoagulation therapy to provide safe and effective medication management.
  • Assist oncology teams with pharmacotherapy prescribing of personalized oncology regimens based on genetic biomarkers.
  • Provide clinical and pharmacotherapy expertise to staff and interdisciplinary members of healthcare team.
  • Aides in protocol development, quality improvement, and patient safety initiatives by participating in multidisciplinary rounds and committee meetings. Education and development of staff:
    • Delivers regular education to pharmacy, nursing, and providers.
    • Pharmacy preceptor for students and residents.
    • Liaises with principal investigators and research assistants to support clinical research trial participation, implementation, and monitoring. Act as resource for oral chemotherapy management for provider or MTM pharmacy if referral placed. This may include following medication through financial approval process but not directly obtaining approvals, selecting appropriate specialty pharmacy prior to release of medication, releasing signed orders after new start counseling or as appropriate for refills, and toxicity management.

Clinical Excellence/Patient Education

  • Engages in routine initiation, follow up, and evaluation of patient outcomes related to medication therapies. Documents all required activities in the electronic medical record utilizing a format that enables outcome assessment and billing.
  • Assists with the medication reconciliation process to avoid inadvertent inconsistencies and medication errors during transitions of care.
  • Provides patient-specific medication counseling to enhance the patient’s understanding of their medications and increase medication regimen adherence.
  • Delivers accurate drug information and experiential education to other health care professionals by keeping up-to-date on the pharmaceutical marketplace.

Qualifications

EDUCATION:

- B.S. Pharmacy with clinical experience required. Doctorate of Pharmacy (PharmD) preferred.

EXPERIENCE:

- A minimum two years hematology/oncology work experience or one year pharmacy residency. Post-graduate year two (PGY2) in hematology/oncology preferred.

LICENSURE/CERTIFICATION:

- Must maintain current license to practice pharmacy in Iowa and be in good standing with the Iowa Board of Pharmacy Examiners to continue employmet required.

- Board Certified Pharmacotherapy Specialist (BCPS) preferred.

- Valid driver's license when driving any vehicle for work-related reasons.

KNOWLEDGE/SKILLS/ABILITIES:

- Must be able to proficiently read, write, and speak English.

- Ability to work with computer systems required.

- Must demonstrate leadership qualities, problem-solving skills and the ability to instruct and lead staff.

- Must demonstarte discretion and sound judgement in all job responsibilities while remaining fair, impartial, and professional.

- Must be able to communicate effectively, both verbally and in writing, with people of diverse professional, educational, and personal backgrounds.

- Must function effectively in fast-paced environment; must be able to quickly alternate among various duties depending on momentary needs.

- Must be able to ascertain and understand pharmacy related problems and provide suitable soloutions through adequate and proper research.

- Must maintain judgement and self-discipline to work under supervision but also possess initiative, innovation, and self-direction.

- Must have ability to maintain a pleasant attitude towards patients, visitors, and fellow employees.
