Manager of Care Coordination
General Summary:
The Manager of Care Management/Social Services provides leadership and operational oversight for the Care Management team, supporting the mission and goals of Mercy Medical Center. This role ensures high‑quality, coordinated care across the healthcare continuum, promotes effective use of resources, and facilitates interdisciplinary collaboration to improve patient outcomes.
Key Responsibilities:
- Lead and support Care Management RNs and Social Workers; promote strong communication and team cohesion.
- Coordinate and evaluate case management and social work services to achieve quality, financial, and patient‑centered goals.
- Oversee interdisciplinary plans of care, ensuring smooth transitions and appropriate resource utilization.
- Address barriers to patient care, including length‑of‑stay and level‑of‑care issues.
- Provide education to patients, families, and staff regarding care transitions and available resources.
- Assist the Director with hiring, evaluations, scheduling, and departmental accreditation requirements.
- Maintain and model clinical expertise while supporting professional development of team members.
- Collaborate with physicians and leadership to collect and analyze data for performance improvement.
Qualifications:
- Minimum 3 years of RN or Social Work clinical experience working with multidisciplinary teams.
- Current Iowa RN license or LISW licensure required.
- Specialty certification preferred.
- Mandatory Reporter training completed within 3 months of hire.
Work Environment:
- Requires regular standing, walking, lifting, and patient interaction.
- Exposure to typical clinical hazards and infection‑control environments.
- Must adhere to HIPAA and organizational standards of conduct.
