Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel.Position Details:
Primary Care RN or LPN - Mercy Hospital Stoddard
 **Must complete three months of on-site training prior to transitioning to hybrid work-from-home schedule; Must also be available to come on-site for monthly clinic meetings and 1-2 times per week**
Our Primary Care Nurse is a newly created role for the clinics. In this position the nurse will work closely with the Providers and manage triage calls/messages for the clinic. They will have the option to move hybrid after training is complete.Â
LPN Qualifications:
Education:Â Diploma in Practical Nursing
Licensure: Active LPN license
Experience: 2-3 years of experience preferred
RN Qualifications:
Education: Associate of Applied Science in Registered Nursing, Graduation from registered nursing program, bachelor’s degree in nursing (BSN), OR 4 years of commensurate experience in lieu of the bachelor’s degree
Licensure: Active RN license
Experience: 2-3 years of experience preferred
