Schedule: Monday–Friday. This is a high-volume practice offering a rewarding, team-oriented work environment.   ESSENTIAL JOB FUNCTIONS:   Advocate for your client and community.  Identify and help address health inequities and barriers to good healthcare.  Meet with clients in their homes and at community events.  Provide social service referrals.  Provide general wellness education.  Transport clients to health-related appointments.  Knowledge of community resources.   Connect patients and families with community resources.   Follow up with clients and agencies as appropriate to document use/success of referral.  Document in the electronic medical records  Assists with follow up and scheduling of appointments as needed.  Completes necessary reports and maintains records of data as required.  Performs other duties as assigned.  KNOWLEDGE, SKILLS, AND ABILITIES:   Valid High School Diploma or GED.  Ability to work independently.  Maintain confidentiality.  Excellent Problem-Solving Skills  Written and verbal communication skills.  Knowledge of community resources and experience with high-risk populations.  Ability to work collaboratively with the community, patients, families and other members of health care teams.    Education, Training and QualificationsEDUCATION, TRAINING AND EXPERIENCE:   Valid High School Diploma or GED.  Valid Driver's License and arability to own insured vehicle.  Familiarity of basic computer skills, word processing, and data entry.   Have experience with basic office skills such as filing, scheduling, and customer service.  Ability to obtain BLS certification.  Willingness to obtain state CHW certification.
