JOB TITLE: Bilingual Patient Navigator and Outreach Specialist- Spanish-Full-time

REPORTS TO: Program Manager, Women’s Health Screening

 

PURPOSE: The Delaware Breast Cancer Coalition is Delaware’s most trusted resource for breast cancer information, education and support.  As a member of the Outreach Team, the Bilingual Patient navigator and Outreach Specialist is responsible for implementing outreach programs to increase breast cancer awareness and screening to the medically underserved and minority communities in New Castle, Kent, and Sussex Counties. In addition, serve as a bilingual interpreter (Spanish) as needed.

 

Salary: This is an hourly position for a work week consisting of 35 hours.  We are currently paid every two weeks.

     

MINIMUM QUALIFICATIONS:

  • BA degree preferred or equivalent work experience in education, health-related field, social sciences, or social work.
  • Bilingual – Spanish language oral and written communication required.
  • Proven community development skills.
  • Knowledge of counties in Delaware and working with diverse communities.
  • Demonstrated competence in Microsoft Office programs.
  • Excellent presentation skills. Ability to coordinate and facilitate learning programs such as Yes2Health along with the ability to teach breast health programs.
  • Excellent interpersonal, writing, collaboration and problem-solving skills. Must be a team-player.
  • Valid driver’s license and excellent driving record required.

 

ESSENTIAL JOB DUTIES:

  1. Outreach and scheduling of all bilingual screenings in the Hispanic communities throughout the State.
  1. Provide referrals to state screening programs, as needed, and to other agency programs (SFL/Survivorship/WHS programs)
  2. Transport patients to mammography appointments as needed.
  1. Provide interpretation for Hispanic clients in need of navigation, during mammograms, and at other times as needed.
  1. Attend community events, health fairs, cultural festivals, etc. to promote breast health awareness and identify those in need of mammography and other preventive screenings.
  2. Promote awareness of agency initiatives in underserved communities within the region.
  3. Assist with the development and implementation of culturally competent breast health programs and mammography screening navigation programs in New Castle, Kent, and Sussex Counties.
  4. Be responsible for building relationships with community leaders and organizations in order to implement the patient navigation program and other DBCC services.
  5. Maintain accurate, up-to-date records of grant-related activities and outcomes and submit completed reports to appropriate individuals, organizations, etc. in a timely manner.
  6. Assist in agency event preparation and participation.
  7. Perform other duties as assigned or deemed necessary.

 

WORKING HOURS: Working hours include business hours Monday - Friday from 9:00 - 5:00. Must also be able to work evenings and weekends depending on the needs of the program and agency events.
