Licensed Practical Nurse (LPN)

Position Summary

The Licensed Practical Nurse (LPN) is responsible for coordinating and overseeing medical care services for individuals supported by the organization. Working as part of a multidisciplinary treatment team, the LPN promotes integrated physical and behavioral healthcare, supports individual recovery goals, manages medication services, and ensures compliance with all applicable regulatory, licensing, and organizational requirements.

Essential Duties and Responsibilities

Clinical Care & Health Management

  • Provide nursing services and health monitoring for individuals receiving services.
  • Assess, monitor, and respond to new, ongoing, and potential health concerns.
  • Coordinate medical care with physicians, prescribers, specialists, and other healthcare providers.
  • Serve as a liaison between individuals, healthcare providers, and community resources to promote overall wellness.
  • Assess medical care needs and recommend health-related objectives for individualized recovery plans.
  • Monitor vital signs, medication effectiveness, and potential side effects, reporting findings to the treatment team as appropriate.
  • Administer medications, including intramuscular (IM) injections, in accordance with physician orders and applicable regulations.

Medication Management

  • Manage the ordering, receipt, storage, administration, and documentation of medications in accordance with program requirements and organizational policies.
  • Collaborate with individuals and treatment teams to develop strategies that improve medication adherence.
  • Ensure individuals maintain prescribed medication schedules and receive appropriate follow-up care.

Assessment & Documentation

  • Conduct screenings and assessments utilizing validated tools for depression, trauma, substance use disorders, and criminal risk/needs factors.
  • Complete assessments, progress notes, recovery plans, treatment contacts, and significant event documentation within established timelines.
  • Utilize collaborative documentation practices and maintain accurate, timely, and confidential records.
  • Ensure compliance with organizational policies and funder, licensing, accreditation, and regulatory requirements.

Education & Training

  • Provide health education and wellness information to individuals receiving services.
  • Deliver nursing-related training and consultation to staff members as needed.
  • Support recovery-oriented and evidence-based treatment approaches.

Care Coordination & Recovery Support

  • Function as an active member of a multidisciplinary team providing holistic, person-centered services.
  • Coordinate primary care, preventive healthcare, annual physical exams, dental care, and other wellness services.
  • Assist individuals in accessing community supports that promote integrated physical and behavioral health.
  • Meet required levels of treatment contacts and service delivery standards.

Crisis Intervention

  • Participate in a rotating on-call schedule to provide 24-hour emergency and crisis intervention support.
  • Respond appropriately to urgent medical and behavioral health situations.

Qualifications

Required

  • Graduate of an accredited Practical Nursing program.
  • Current Licensed Practical Nurse (LPN) licensure in the State of Delaware.
  • Valid driver's license with an acceptable driving record.
  • Knowledge of medication administration, health assessment, and care coordination practices.
  • Strong communication, documentation, and organizational skills.
  • Ability to work effectively within a multidisciplinary team.

Working Conditions

This position may involve exposure to:

  • Dust, fumes, mists, and gases
  • Extreme heat and cold
  • Wet and/or humid environments
  • Loud noise levels
  • Confined spaces
  • Mechanical, chemical, electrical, and burn hazards

The employee may be required to travel within the community to support individuals receiving services and coordinate medical care.
