What You Bring

  • Minimum: HS Diploma + 12 college credits in a related field
  • Preferred: Bachelor’s degree in psychology, social work, counseling, or related field
  • 1+ year of direct client care experience (internships/volunteer work considered)
  • Experience with mobile services and case management preferred
  • Strong communication and computer skills
  • Valid Driver’s License
  • Ability to meet physical requirements of the role, with or without accommodation

 

Your Role

  • Provide community-based Blended Case Management services to children and adults in Central Bucks County
  • Meet individuals and family members in their homes and communities to assess needs and connect them to vital services, resources and programs to positively impact an individual’s overall well-being
  • Collaborate with LVF’s multidisciplinary team and external providers
  • Maintain accurate, recovery-oriented documentation in our Electronic Health Record (EHR)
