Description

This position is for a Care Coordinator who supports the mission of “Saving Lives, Instilling Hope and Restoring Relationships” in an integrated team with patients receiving counseling, case management and medically assisted treatment. **This position will support our Dublin, Norton and Galax clinics**


The Care Coordinator's responsibilities include, but are not limited to the following:

  • Assess patient needs and barriers
  • Develop and implement the treatment plan in collaboration with the patient
  • Advocate on behalf of the patient for needed services and benefits
  • Connect patients with appropriate resources and assist them with obtaining their needs such as transportation, medical assistance, etc.
  • Provide education, modeling, and support in the areas of daily living, budgeting, money management, and medication management
  • Engage patient in recovery-oriented, patient centered care in accordance with professional standards of practice
  • Complete required Documentation in an appropriate and timely manner
  • Ensuring compliance with HIPPA and CFR 42 part 2 regulations
  • Collaboration with all members of the multidisciplinary team to enhance patient experience and continuity of care
  • Community Outreach as applicable.

Requirements

Education:

  •  Bachelor’s degree in one of the following fields (social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation, human services counseling)


OR

  •  Board of Counseling Certified Substance Abuse Counselor (CSAC) or CSAC-Assistant under supervision as defined in 18VAC115-30-10 et seq.

Experience:

  •  One year of substance use related direct experience providing services to individuals with a diagnosis of substance use disorder.

OR

  •  A minimum of one year of clinical experience working with individual with cooccurring Diagnoses of substance use disorder and mental illness.
