The SLP will assess, plan, and implement speech therapy services to help patients achieve their highest level of independence and communication ability in the home setting. Key Responsibilities ✅ Provide effective patient care using approved techniques and procedures ✅ Evaluate patient’s condition, functional level, and therapy goals — maintaining accurate records ✅ Develop and carry out individualized treatment plans in coordination with the care team ✅ Provide therapy services as outlined in the Plan of Care ✅ Assist patients and families with home programs and adaptive equipment recommendations ✅ Document all visits, progress, and discharge summaries in accordance with HIPAA standards ✅ Communicate and collaborate with the Director of Clinical Services, nurses, and physicians to ensure coordinated patient care ✅ Evaluate the effectiveness of therapy and adjust treatment plans as needed Qualifications Qualifications ✔ Active Ohio SLP license ✔ Minimum 1 year of SLP experience (home health preferred) ✔ Physical exam within 6 months prior to first patient contact ✔ Current TB test (within the last 12 months) ✔ Current CPR certification ✔ Ability to attend 1-day orientation at our Dublin office Why Join Aspire Home Healthcare?
