We’re looking for a Social Worker (BSW) to join our hospital team and provide meaningful support to patients and families during some of life’s most challenging moments. Responsibilities
- Provide social work support, assessment, and resources to patients and families.
- Help patients navigate healthcare, community resources, financial assistance, and other support services.
- Collaborate with nurses, providers, case management, and the interdisciplinary care team.
- Assist with discharge planning and transitions of care.
- Advocate for patient needs while promoting dignity, independence, and well-being.
- Provide emotional support, education, crisis intervention, and referrals as appropriate.
Qualifications
Education:Â
- Bachelors Degree in Social Work from an accredited school of Social Work
Experience:Â
- One year of clinical experience in health care preferred, but not required
