Job DetailsLevel: EntryJob Location: Duluth Office - Duluth, MN 55806Position Type: Full TimeEducation Level: 4 Year DegreeSalary Range: $48,000.00 - $56,000.00 Salary/yearJob Shift: Day*Per state guidelines, this position requires a 4 -year degree in Social Services or related field OR 4-year degree in unrelated field plus one year of social work/care coordination.  We do not require any additional experience beyond what is required by state guidelines   YOU WOULD BE A GOOD FIT IF YOU:      Desire to be a new member of a supportive team!  Value autonomy to prioritize and complete tasks by required deadlines.   Engage naturally in personal connection while maintaining appropriate boundaries.   Are a resourceful and proactive problem solver.   Adapt to the changing needs of a diverse caseload and county policy changes.     SCHEDULE AND COMPENSATION:    Monday – Friday, daytime hours  Hybrid - eligible to work from home up to two days per week after successful completion of the 90-day introductory period  Hourly Rate - $23-$27 depending on experience and qualifications   $23 - Very little-to-no experience in human services   $24.50 - Experienced waiver case manager or experienced human services professional   $26+ - 5+ years of experience as waiver case manager or 10+ years as a human services professional  Career path - opportunities for promotion and compensation increases at 6 months, 1 year, 2 years and beyond!   Annual anniversary pay increases  Benefits - Health, dental, life insurance and 401K (eligible at 90 days of full-time employment), as well as PTO and paid holidays.  Mileage reimbursement      REQUIRED QUALIFICATIONS:     To be eligible to be a Case Manager you must have a bachelor’s degree in health and human services OR bachelor’s degree in any field with 1-year of experience in the human services field  Ability to pass DHS background check   Driver’s license, access to a reliable vehicle, vehicle insurance & a clean driving record     MORE ON THE ROLE:      You'll be on a team based out of our headquarters in Saint Paul in a satalite office in Duluth. Some of the duties within the case manager role include:  Serve a caseload of clients – our average is 38 clients per caseload, lower than industry standard!  Complete new client intakes  Develop initial service plan with the client and legal representative  Coordinate services between providers and clients  Schedule in-person meetings with clients at least twice per year and make monthly phone contact with each client  Maintain quality documentation, such as: case notes, service plans and service agreements.  With each client, you will work to uncover their needs and aspirations, and work on connecting them to the appropriate services and resources.  You'll be a liaison for your client to ensure that their whole care team is well aligned, each doing their part to support their goals and meet their needs.     What waivers or programs do we work with?  AC - Alternative Care Program  BI - Brain Injury Waiver  CADI - Community Access for Disability Inclusion Waiver  DD – Developmental Disabilities Waiver, Non-Waiver  EW – Elderly Waiver      WHY SLS?  We are about building careers and a thriving community!    At SLS you can:   Make an impact: We are driven to make a difference!
