SUMMARY:

The LPN provides support to Gateway’s high acuity adult programs under the umbrella of Gateway’s nursing department and under supervision of program RN. 

The position has two primary functions:

  • 1. promote the overall psychiatric and physical health of clients through screenings and coordination of care with primary and specialty care providers, monitoring targeted health indicators; and

  • 2. provide support to RNs and other team members in monitoring caseloads and required documentation, communicating with clients and collateral contacts on appointments, medications, lab work, and referrals.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Completes required screenings for general health, including sleep issues, asthma, oral health, immunizations, tobacco use and exposure, communicable diseases and other infectious diseases.

  • Completes screenings using standardized tools for substance use and mental health, including SBIRT, trauma, depression, anxiety, suicidal ideation, and social determinants of health, contributing information to the person-centered treatment planning process

  • As part of the client’s care team, coordinates care with community primary and specialty care providers to support integrated care for clients, maintaining frequent and timely communication.

  • Provides relevant education to clients and collateral contacts about medical and psychiatric conditions.

  • Meets with clients and support people in community to perform job responsibilities.

  • Engages and maintains collaborative, positive relationships with a network of specialized community providers to address needs of population, including nutrition education, physical activities, smoking cessation, health literacy, housing, social and rehabilitative programs, and job training and placement

  • Maintains detailed, accurate, and timely documentation in medical record.

  • Contributes knowledge and expertise to the teams.

  • Attends supervision, agency, and team staff meetings as scheduled.

  • Enhances and maintains professional expertise through educational activities and trainings.

  • Engages in trainings on evidence-based practice models specific to their assigned population, including motivational interviewing, implementing learned skills in daily interventions.

  • Follows agency policies and standards for client rights and confidentiality

  • Maintains practical nursing professional standards of care.

  • Performs other duties as assigned.

QUALIFICATION REQUIREMENTS:

  • Graduate from an accredited school of practical nursing with current license to practice as a Licensed Practical Nurse (LPN) in the State of Rhode Island

  • Preference for an LPN with experience in behavioral health setting

NECESSARY SPECIAL REQUIREMENTS:

  • Valid driver's license and use of car to complete major functions of job

  • Automobile liability insurance coverage for vehicle being used in accordance with agency requirements

  • CPR training within 30 working days of employment and maintained annually

JOB KNOWLEDGE, SKILL, AND ABILITY:

  • Understanding of mental health and substance use diagnoses, including severe mental illness

  • Knowledge of community resources

  • Familiarity with screening/ assessment tools and evidence-based practices relevant to position

  • Oral and written communication skills at a proficient level, ensuring effective and professional communication

SUPERVISORY RESPONSIBILITY:

None.

Pay Range:

$24.29-$40.07
